If you are a new patient to our office, please visit our patient forms page to download our new patient form.  For your convenience you can download the form by clicking the button below and print it out to complete the from home.  Then just bring your completed for with you when you come in for your appointment.  This makes it easier for you and allows you to relax when you come in for your appointment.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
